Individual: $89 monthly for ages 5-64 and $119 monthly for ages 65+ with an initial $150 enrollment fee

Family Plan (parent(s) and dependent children ages 5-18 within the same household): $300 monthly with an initial $400 enrollment fee

Business Plan: $89 monthly with an initial discounted $75 enrollment fee per employee (with enrollment of 10 or more employees)
